To submit your case please fill out the form below.  Please try to be as specific as possible regarding your disorder or diagnosis and the connection you have made with the information in the book.  Cases may be used in presentations and possibly future publications. Names will be kept anonymous as will any clear identifying information. If you do not wish to have your case used in this way please clearly note this in your submission below.
